April 2013- This plaque from the Tomah VA Medical Center was presented to Scenic Bluffs Community Health Centers with appreciation for expanding behavioral health services to veterans in the area. This is part of the VA's Community Partnership Program that is being piloted with only 15 Community Health Centers across the nation.

Patient Centered Medical Home Recognition
Scenic Bluffs Community Health Centers was awarded the highest level of
recognition as a Patient Centered Medical Home (PCMH). This recognition comes after many months of effort and is not easily obtained. “We are part of only 18% of community health centers recognized nationally, and only the second in the state of Wisconsin,” stated Executive Director Mari Freiberg.
PCMH is a primary care model that offers services that are relationship-based with an orientation toward the whole person. The patient centered healthcare home is accountable for meeting the large majority of each patient’s physical and mental healthcare needs, including prevention and wellness, acute care, and chronic care.
The Scenic Bluffs system coordinates care across the broader healthcare system,
including specialty care, hospitals, home health care, and community services and supports. Said Freiberg, “We are committed to quality, and quality improvement;delivering evidence-based care, measuring performance, tracking and responding to patient experiences, and practicing population health management.”
Scenic Bluffs currently employs over seventy staff members throughout the two clinical locations in Cashton and Norwalk. This official acknowledgment identifies those seventy individuals as a healthcare team. Being a multi-practice health center (medical, dental, behavioral health, chiropractic and pharmacy) is a major factor contributing to this achievement. Physical proximity and a mentality of working across the hallway enables Scenic Bluffs’ employees to offer patients an integrative medicine program, timely services and relevant health education.
This recognition of PCMH comes from the National Committee for Quality Assurance. As
healthcare topics dominate the news, PCMH recognition underscores the need to focus
on effective primary care delivery systems. The focus is not the building or the staff but

Scenic Bluffs has been recognized by the US Department of Health and Human Services, Center for Quality and the National Institutes of Health for excellence in primary care for people with chronic health conditions-diabetes and cardiovascular disease. The Health Center was one of 25 selected to meet with a team of federal officials about successful methods we have instituted to achieve improved health outcomes for our patients with these illnesses. The meeting was designed to share "best practices" and advance information on how other health care organizations can achieve and maintain excellence in care for populations. This winter Scenic Bluffs Community Health Centers was recognized at the BadgerCare Plus Heroes Awards Banquet that was held at the Governor’s conference room in Madison, WI.
Governor Jim Doyle, Department of Health Services Secretary Karen Timberlake and Wisconsin Medicaid Director Jason Helgerson recognized the organization for all it has done in helping Wisconsin families and individuals gain access to health care, specifically through their efforts to provide one-on-one application assistance.
Governor Jim Doyle thanked these organizations, including Scenic Bluffs, for their work in helping enroll more than 15,000 new families to BadgerCare, Medicaid, and other state aid programs in 2010.
